The permit

Five different people: requestor · HSE (plant, corporate, admin) · area owner (plant, admin) · supervisor (plant, site, admin) · approver (none of the other four).

StepNeeds
Approve3 signatures · approver's 4 ticks (JHA, isolation plan, team, PPE) · photos: JSA, HIRA, toolbox talk
ActivateApproved JHA · an end time · photos: locks and tags, tools and test equipment · high risk: JSA, arc-flash PPE, LOTO, certificates ticked by the person activating
CloseLocks removed first · closure notes

Every photo different. Expired permit: extend (reason + later end time) or raise a new one before work restarts.

Locks (LOTO)

  1. Fit locks, record each point, lock, tag and the fitter → pending verification. Fitter and form filer both count as installer.
  2. Someone who is neither proves zero energy, a reading for every point → active.
  3. At the end, someone else removes them, verifier neither fitter nor filer → removed. Emergency removal: plant manager of that plant, corporate or admin.

End of the job, in order

Complete (checklist, statements, notes, downtime hours) → verify (failure, cause, remedy codes) → locks off → permit closed → work order closed.

Emergency override

Corporate or admin only · reason of 40 characters or more · never the assigned technician · one step · 4 h default, 8 h maximum · recorded, HSE notified · used up even if a later check refuses the step, so fill in the checklist first.
